Crown Lengthening: Surgical and Esthetic Considerations

Crown lengthening is a periodontal surgical procedure that removes gum tissue and, when necessary, bone to expose more of the tooth. It is performed for two main reasons: to provide enough tooth structure for a restoration, and to improve the appearance of a gummy smile.

Why Crown Lengthening Is Performed

The most common indication is restorative. When a tooth is decayed or fractured below the gum line, or when there is insufficient clinical crown to retain a restoration, crown lengthening exposes additional sound tooth structure so that a crown or filling can be placed with a proper margin.

The second indication is esthetic. Patients with a gummy smile, in which excessive gingiva covers the teeth, may undergo esthetic crown lengthening to create a more balanced, harmonious smile. The two goals require different amounts of tissue removal and different surgical planning.

The Biologic Width

Biologic width is the dimension of the junctional epithelium and connective tissue attachment that lies between the gingival margin and the alveolar bone, typically about two to three millimeters. If a restoration margin is placed within this zone, chronic inflammation, bleeding, and bone loss are likely to follow.

Crown lengthening therefore aims to create enough space between the planned restoration margin and the bone crest to respect the biologic width. This is why bone removal is often necessary even when the visible problem appears to be only excess gum tissue.

A gingivectomy removes excess soft tissue without raising a flap and is suitable when only gingival overgrowth is present. When bone must be removed to establish the biologic width, an apically positioned flap is raised, the bone is recontoured, and the flap is sutured at a new level.

Esthetic Crown Lengthening

Esthetic crown lengthening is planned with the smile in mind. The goal is to create a gingival contour that is symmetric and harmonious, with the gingival margins of the maxillary anterior teeth following the curve of the upper lip. The procedure is often combined with restorative work such as veneers or crowns.

Careful planning is essential, since removing too much tissue can expose the roots and create an unesthetic result, while removing too little fails to achieve the desired improvement. Digital smile design and diagnostic wax-ups help guide the surgical plan.

Healing and Timing of Restoration

After surgery, the gingival tissues heal and mature over several weeks to months. The final gingival margin position is not stable immediately, and placing a permanent restoration too early can result in an ill-fitting margin. A provisional restoration is often worn while the tissues settle.

For restorative crown lengthening, the final impression is usually taken after a healing period of six to eight weeks, when the gingival contour has stabilized. In the esthetic zone, longer healing may be required to achieve optimal soft tissue contours.

Risks and Considerations

Crown lengthening carries the risk of exposing root surfaces, which can cause sensitivity and, in the esthetic zone, an unappealing appearance. In multi-rooted teeth, furcation involvement may be exposed, complicating hygiene and restoration. The procedure is not suitable when insufficient attached gingiva or inadequate bone remains.

Before surgery, the clinician must assess the crown-to-root ratio, the position of the bone, and the amount of tooth structure that can be exposed without compromising the tooth's support. With careful planning, crown lengthening reliably improves both the restorability and the appearance of the dentition.

Preoperative Assessment

A thorough preoperative evaluation is essential. The clinician must assess the amount of remaining tooth structure, the position of the gingival margin, the level of the alveolar bone, and the crown-to-root ratio. Radiographs, including periapical films and sometimes CBCT, help determine how much bone must be removed.

In the esthetic zone, a diagnostic wax-up and a trial restoration allow the patient to preview the planned result. The surgical plan should specify the desired gingival margin position and the amount of bone recontouring required to achieve it while respecting the biologic width.

Postoperative Care and Outcomes

After surgery, patients are advised to avoid brushing the surgical site for the first week and to use a chlorhexidine rinse. Analgesics and, when indicated, antibiotics are prescribed. Sutures are removed after one to two weeks, and the patient is seen for follow-up to monitor healing.

With proper planning and technique, crown lengthening achieves predictable results. Restorative cases gain the tooth structure needed for a durable restoration, and esthetic cases achieve a more balanced smile. Long-term success depends on maintaining periodontal health and appropriate restorative care.

Combining Crown Lengthening with Other Procedures

Crown lengthening is often combined with other procedures to achieve the best result. In the esthetic zone, it may be performed together with gingival grafting to augment soft tissue, or with orthodontic treatment to reposition the teeth before surgery. In restorative cases, it may be coordinated with endodontic treatment or the placement of a new restoration.

When multiple procedures are planned, sequencing is critical. Orthodontic movement is usually completed before crown lengthening, and the final restoration is placed only after the tissues have fully healed. A coordinated, interdisciplinary approach ensures that each procedure supports the overall treatment goal.
