After root canal treatment, a tooth may retain insufficient coronal structure to support a crown. A post and core restoration provides the foundation needed to retain the final restoration, but it must be planned carefully to avoid weakening the root and causing fracture.
A post is indicated when there is inadequate remaining coronal tooth structure to retain a core and the final crown. The post does not strengthen the tooth; in fact, it slightly weakens it by removing root dentin. Its only purpose is retention of the core.
If sufficient coronal tooth structure remains, a post should be avoided entirely. Modern adhesive dentistry allows many endodontically treated teeth to be restored with a bonded core and crown without any post, preserving maximum tooth structure and reducing the risk of root fracture.

Fiber posts have become popular because their flexibility is similar to dentin, which distributes stress more favorably and reduces the risk of catastrophic root fracture. Metal posts are stronger but transmit more stress to the root, increasing fracture risk in weakened teeth.
The post space is prepared by removing gutta-percha from the canal, leaving an apical seal of at least four to five millimeters. The post should be as long as possible while preserving this seal, and the post diameter should be minimal, removing as little dentin as possible.
Excessive post length or diameter significantly weakens the root. The ferrule effect is critical: a crown that encircles at least one and a half to two millimeters of sound tooth structure above the margin dramatically reduces the risk of fracture. Whenever possible, a ferrule should be created.
The ferrule is a band of remaining tooth structure that surrounds the crown margin. It acts like a metal ring around a wooden barrel, resisting the wedging forces that would otherwise split the root. A proper ferrule is one of the strongest predictors of long-term success in post-retained restorations.
When insufficient coronal structure remains to create a ferrule, crown lengthening or orthodontic extrusion may be considered to expose additional sound tooth structure before restoration.
The core replaces missing coronal tooth structure and provides retention for the crown. Composite resin is the most common core material because it bonds to both the post and the remaining tooth, is easy to manipulate, and can be built up in a single visit. Amalgam cores are less common today but remain an option in posterior teeth.
Glass ionomer and resin-modified glass ionomer cores are used in some situations, though they are generally weaker than composite. The choice of core material should consider the amount of remaining tooth structure, the post type, and the planned final restoration.
The procedure begins with assessment of remaining tooth structure and confirmation of an adequate apical seal. The post space is prepared, the post is tried in, and the canal is cleaned and dried. The post is cemented with an appropriate luting agent, and the core is built up around it.
After the core is shaped and finished, the tooth is prepared for the final crown, and an impression is taken. The definitive restoration is then fabricated and cemented. Throughout the process, preservation of tooth structure and creation of a ferrule remain the guiding principles.
The most serious complication of post-retained restorations is root fracture, which usually results in extraction. Other complications include post loosening, core fracture, and recurrent caries. Fiber posts that loosen can often be recemented, whereas a fractured root is generally non-restorable.
With careful case selection, adequate ferrule, and conservative post preparation, post and core restorations have a high long-term success rate. The key message is that a post is a means of retention, not reinforcement, and the preservation of sound tooth structure is always the priority.
The success of fiber posts depends heavily on adhesive luting. The canal is etched, primed, and bonded, and the post is cemented with a resin cement that forms a strong bond to both the post and the root dentin. This creates a monoblock that distributes occlusal forces more evenly along the root.
Moisture control is challenging in the deep canal, and the bond to root dentin is less predictable than coronal bonding. Techniques such as the use of a bonding agent with a low film thickness and careful light curing help improve the adhesive interface and reduce the risk of post debonding.
In many cases, a post can be avoided altogether. When adequate coronal tooth structure remains, a bonded composite core and crown provide excellent retention without the risks associated with post preparation. Adhesive techniques have made post-free restorations increasingly predictable.
For teeth with minimal remaining structure, alternatives such as adhesive onlays, endocrowns, or, in severe cases, extraction and implant placement should be considered. The decision should weigh the restorability of the tooth, the quality of the remaining structure, and the long-term prognosis of each option.
