Lingual Arch and Nance Appliance

The maxillary equivalent, the Nance appliance, adds an acrylic button on the palate for additional anchorage. These appliances are effective but require careful oral hygiene because food accumulates around the components.

Distal Shoe

The distal shoe is a specialized appliance used when the second primary molar is lost before the permanent first molar has erupted. Its distal extension rests in the soft tissue at the mesial of the developing first molar, guiding it into proper position. It is technically demanding to fit and must be monitored closely.

Selection Criteria

The selection of a space maintainer depends on the number of missing teeth, the arch, and the stage of development. Single-tooth losses in the posterior region are managed with band and loop or crown and loop appliances, while multiple losses favor arch appliances. Anterior losses, which are less critical for space preservation, may be managed with a removable appliance when function or esthetics requires it. The clinician should also consider the presence of caries on the abutment teeth, since an adequate anchor tooth is essential for a fixed appliance.

Care and Complications

Space maintainers require meticulous care and regular review. Food impaction around the band and wire is common and can cause gingivitis, caries, and decalcification if plaque is not controlled. Parents should be instructed on brushing around the appliance and limiting sticky foods that could dislodge it.
