Why Over-Brushing and Mouthwash Abuse Can Disrupt Your Oral Microbiome Balance

口腔微生物组:一个微妙的生态系统

口腔内栖息着超过700种细菌、真菌和病毒,它们共同构成了口腔微生物群。在健康状态下,这个微生物群落处于平衡状态——有益菌抑制致病菌的过度繁殖,而免疫系统则维持着一种可控的、低水平的炎症反应,防止微生物过度生长。然而,这种平衡却极其脆弱。

现代口腔卫生习惯如果走向极端,反而会破坏口腔生态系统,反而增加患口腔疾病的风险。过度刷牙和过度使用漱口水是其中两个最常见也最容易被忽视的罪魁祸首。

过度刷牙的问题:并非越多越好

刷牙用力过猛或过于频繁会导致牙刷磨损。这种机械力不仅会磨损牙菌斑,还会磨损保护性的唾液膜——这层薄薄的蛋白质膜覆盖在牙釉质表面,是抵御酸性物质侵蚀的第一道防线。没有了这层保护膜,牙釉质就会直接暴露于食物中的酸和细菌代谢产物中。

随着时间的推移,用力刷牙会侵蚀覆盖牙根的牙骨质,使下方的牙本质暴露出来。牙本质中含有数千条微小的牙本质小管,这些小管直接通向牙神经。一旦暴露,冷热和甜味刺激都会引发剧烈疼痛——这是牙本质过敏的典型症状。2019 年发表在《临床牙周病学杂志》上的一项研究发现,与每天刷牙两次的人相比,每天刷牙三次以上的人牙龈萎缩的发生率显著更高。

这种损害不仅限于牙齿。用力刷牙会损伤牙龈上皮,导致微溃疡,使口腔细菌得以进入血液。这种短暂的菌血症通常无害,但对于免疫系统受损或患有心脏瓣膜异常的人来说,则可能导致严重的全身感染。

漱口水:一把双刃剑

抗菌漱口水,特别是含有氯己定或西吡氯铵的漱口水,如果使用得当,能有效减少牙菌斑和牙龈炎。但问题在于,如果每天长期使用,超过氯己定推荐的两周疗程,就会产生不良反应。

这些广谱抗菌剂无法区分致病菌和有益菌。它们会清除舌头上的硝酸盐还原菌——这些微生物能将蔬菜中的硝酸盐转化为亚硝酸盐,亚硝酸盐随后被吞咽并在胃中转化为一氧化氮。一氧化氮是一种重要的信号分子,能够调节血压、改善内皮功能并增强运动表现。多项研究表明,长期使用抗菌漱口水与血压升高有关。其中一项发表于2017年《自由基生物学与医学》的研究报告称,每天两次使用氯己定漱口水仅一周后,收缩压就升高了2至3.5 mmHg。

含酒精的漱口水会带来额外的担忧。许多市售产品中常见的20%至26%的乙醇浓度会导致上皮脱落,破坏口腔黏膜细胞的保护性外层。这种慢性刺激被认为(尽管尚未得到确凿证实)会与其他风险因素(例如吸烟和饮酒)共同作用,增加口腔癌的风险。

重建平衡:一种更明智的方法

恢复口腔微生物群平衡需要克制。每天用软毛牙刷轻轻刷牙两次。抗菌漱口水应在牙医指导下短期用于治疗,例如口腔手术后、急性牙龈炎发作期间或暂时无法通过机械方法清除牙菌斑时。日常使用时,建议选择不含酒精的配方,或直接用清水漱口。

饮食选择也很重要。富含植物性硝酸盐(如绿叶蔬菜、甜菜、芹菜)的饮食有助于维持口腔微生物群落的平衡,而这些微生物正是漱口水会破坏的。益生菌食品和发酵食品可能有助于丰富口腔微生物群,但该领域的研究仍在进行中。

口腔并非无菌环境,也不应该是。口腔卫生的目标并非彻底清除所有细菌,而是维持一个稳定且有益健康的微生物群落。更科学地刷牙(而非用力刷牙)并合理使用化学清洁剂,是这一理念的基础。
