Complete denture fabrication has been a cornerstone of prosthodontic practice for over a century, restoring function and aesthetics to the edentulous patient. While the fundamental principles of retention, stability, and support remain unchanged, the methods by which complete dentures are fabricated have evolved dramatically with the introduction of digital technologies. Understanding both conventional and digital workflows allows clinicians to select the most appropriate approach for each patient.

Conventional Complete Denture Fabrication

The conventional pathway begins with preliminary impressions, typically taken with stock trays and irreversible hydrocolloid (alginate) material. These primary impressions are poured in dental stone to produce diagnostic casts, upon which custom impression trays are fabricated using autopolymerizing acrylic resin. Border molding with modeling compound or polyvinyl siloxane materials captures the functional depth and width of the sulcus, after which a definitive impression is made using zinc oxide eugenol paste, polysulfide, or addition silicone materials.

Jaw relation records constitute a critical phase in complete denture construction. The maxillomandibular relationship in three dimensions—vertical dimension of occlusion, centric relation, and the horizontal orientation of the occlusal plane—must be accurately recorded and transferred to an articulator. Occlusion rims fabricated on stabilized record bases serve as the vehicle for these records. The dentist also evaluates lip support, smile line, midline, and canine line during this appointment, integrating aesthetic parameters into the functional framework.

The try-in appointment allows verification of all prior records in the patient's mouth. Teeth arranged in wax on trial denture bases are evaluated for aesthetics, phonetics, and occlusal harmony. Adjustments made at this stage prevent costly remakes after processing. Following the try-in, the dentures are flasked, packed with heat-polymerized polymethyl methacrylate (PMMA) resin, and processed in a water bath using a controlled curing cycle to minimize porosity and dimensional change. After deflasking, the dentures are finished, polished, and delivered to the patient with appropriate post-insertion instructions.

Digital Complete Denture Fabrication

Digital workflows for complete dentures have matured significantly over the past decade, offering advantages in speed, reproducibility, and reduced chair time. Two primary digital pathways exist: the fully digital approach and the hybrid approach that combines digital and conventional steps.

In the fully digital workflow, intraoral scanning of the edentulous arches replaces conventional impressions. However, scanning mobile, resilient mucosa presents unique challenges, and specialized scanning protocols involving multiple overlapping scans are required. Some systems circumvent this by scanning the denture-bearing area after strategically placed tissue modifications or by using scan bodies on stabilized record bases. The resulting digital model is then used to design denture bases and tooth arrangements using CAD software.

Digital denture bases can be manufactured either by subtractive milling from pre-polymerized PMMA discs or by additive manufacturing using 3D printing technologies. Milled dentures benefit from the superior material properties of industrially polymerized PMMA, which exhibits higher density, lower residual monomer, and improved color stability compared to conventionally processed acrylic. 3D printed denture bases, while rapidly evolving, still face challenges with long-term color stability and wear resistance. Denture teeth are either milled from multilayer PMMA blocks, attached mechanically to the base, or printed in place using multi-material printing.

The Hybrid Approach: Best of Both Worlds

Many clinicians have adopted a hybrid workflow that leverages digital technology for specific steps while retaining conventional methods where they remain superior. A common hybrid protocol uses conventional border-molded impressions to capture functional tissue anatomy while employing intraoral scanning for the definitive impression and jaw relation records. The digital data is then used to design and fabricate trial dentures via CAD/CAM, which are tried in conventionally. Final dentures may be milled or processed using traditional methods from the verified digital design.

The hybrid approach addresses the primary limitation of fully digital workflows—the difficulty of capturing dynamic tissue movements—while still benefiting from the reproducibility, archiving, and fabrication efficiency of digital technology. Should a denture require replacement, the digital design file allows for rapid duplication with minimal clinical steps.

Clinical Outcomes and Patient Satisfaction

Studies comparing digital and conventional complete dentures report comparable patient satisfaction and clinical outcomes when both are executed competently. Digital dentures offer distinct advantages in situations requiring rapid turnaround, such as immediate dentures for patients undergoing extractions on the same day. The ability to store and retrieve digital records simplifies remakes and future replacements. However, the higher initial investment in intraoral scanners, CAD software, and manufacturing equipment must be weighed against the potential efficiencies gained.

Ultimately, the choice between conventional, digital, and hybrid workflows should be guided by patient-specific factors including ridge morphology, mucosal resilience, neuromuscular control, and aesthetic expectations, as well as the clinician's training and available technology. The goal remains the same regardless of the tools employed: to provide the edentulous patient with comfortable, functional, and aesthetic prostheses that restore quality of life.
